Parish of the Holy Sacrifice

We are getting married this year and one of the usual things that any couple would think of is the Wedding Venue. Among the many Churches that we first thought of was the Parish of the Holy Sacrifice at the University of the Philippines Campus. Its complete address is J.P. Laurel cor. Apacible Streets, Diliman, Quezon City 1101 and can be contacted at telephone nos. 926-9496, 926-1592. We visited the Parish Office, which operates at 8:00am – 12:00pm, 1:00pm – 5pm on Tuesday until Sunday, to inquire about the requirements. Documents to be submitted will come from the Church, the Local Civil Registrar and Philippine Statistics Authority (former NSO).

From the Church (Parish of the Holy Sacrifice):

-Baptismal & Confirmation Certificate ~ Original copy; notation (“For Marriage Purpose”); issued within three (3) months before the wedding date. Required to both parties.
-Banns ~ Reply form duly accomplished and signed by the Pastos of respective parties.
-Permission of Proper Pastor ~ The bride’s pastor shall issue permission allowing solemnized outside the bride’s parish.
-Picture of Bride and Groom 2×2 ~ 2 sets for filing and posting in the parish bulletin.

From the Philippine Statistics Authority (PSA)

-Authenticated or Certified True Copy of Birth Certificate
-Certificate of No Marriage (CENOMAR)
-Marriage Contract ~ a certified true copy (for those civilly married)
-Death Certificate ~ widows/widowers are required to submit an authenticated Death Certificate of their deceased spouse

From the Local Civil Registrar

-Marriage Licence
